1. Medical History and Training Load
Workload, among other factors, often contribute to hip flexor tightness.
- Recent increases in training. Have you increased your mileage, intensity, or training sessions?
- Previous injuries. Any recent injuries? Or in the past?
- Sport-specific demands. What motions do you perform frequently as you play or work out?
- Pain patterns. Where is the pain centered? Does it increase during or after playing?
- Recovery habits. Do you take rest days? How are you sleeping?
2. Standing Posture Assessment
How you stand, walk, and sit can affect your hip mobility. It also affects your gait, which could lead to muscle imbalances or stress on the hip or other joints.
Therefore, a physical therapists evaluate:
- Pelvic position. Do your hips and pelvis tilt forward?
- Lumbar spine posture. Does your lower back look straight without a natural curve and do you stoop forward?
- Hip alignment. Do you lean to one side when you stand?
- Weight distribution. Does your weight appear to slant to one side more than the other?
3. Hip Range of Motion
High-level athletic performance depends on full range of motion in the hip. Any limitations can decrease execution and increase injury risk.
To assess mobility, a physical therapist will examine:
- Hip extension

- Internal rotation
- External rotation
4. Muscle Length Testing
Muscle length testing is different from muscle strength screening or range of motion assessment. Rather, this exam lengthens the muscle in the opposite direction to its action to indicate if the muscle is short, long, or normal.
Orthopedists and physical therapists typically use the Thomas Test to assess muscle length.
- The patient lies on their back on an examination table with their legs hanging off the table.
- The patient pulls one knee toward their chest and holds it with both hands.
- The physician or physical therapist will observe the hip and knee position of the lowered, dangling leg.
The position of the thigh or the extension of the knee of the lowered leg as it hangs off the table can indicate which of these muscles are short or tight:
- Hip flexors
- Quadriceps
- Hamstrings
- Adductors or glutes
- Tensor fascia latae (TFL). The muscle at the front of your hips at both sides of the pelvis. It attaches the gluteus maximus to the IT band.
5. Strength Assessment
A complete hip assessment by a PT will evaluate all muscles in the pelvic area listed below. Glute strength, in particular, is important because weak glutes force the hip flexors at the front of the hips to overcompensate. The hip flexors respond by tensing up.
